Personal Support Worker (PSW)

Job summary

A Personal Support Worker provides compassionate and heartfelt personal care services to clients and assists clients with the activities of daily living while maintaining autonomy.

✓ Provide personal care service according to direction of the established care plan and ensure to continuously monitor the emotional, spiritual, functional, and physical comfort and safety of our clients
✓ Provide personal care that is within the provincial scope of practice for a Personal Support Worker: Duties may include skin care, hair care, mouth care, bathing, bowel and bladder care, positioning and movement, exercise, basic wound care, feeding including special diets, assistance with oxygen administration and medication reminder
✓ Understands and implements infection prevention practices
✓ Adhere to Canadian Shield Health Care Services Policies and Procedures

Registered Nurse & Registered Practical Nurse

Job summary

The Registered Nurse provides high quality professional nursing care to assigned patients. The RN’s responsibilities include providing the full range of assessment, treatment, education, provision of health promotion and disease prevention strategies, performing technical nursing interventions, case management, and completion of documentation.

✓ Performs nursing assessments, nursing treatments, health education/counselling, and other clinical activities within the scope of practice for RNs
✓ Participate in the development, implementation, monitoring and evaluation of treatment, education and health promotion programs and services for our clients and their families
✓ Plan of care may include but is not limited to: initial nursing assessment of new clients, monitoring client’s chronic illnesses, client education, individual and group counselling, appropriate referrals

Care Coordinator

Job summary

The Care Coordinator handles client care and service inquiries, arranges client care and services, and schedules our field staff employees in a way that ensures continuity of care and client preference.

✓ Assess client service needs in consultation with the Supervisors; scheduled employees for care and service assignments; promote consistency of caregiver assignments and coordination of services
✓ Reports and collaborates with other case managers, funder or government program as required, along with management
✓ Complete data entry and maintain accurate current scheduling documentation
✓ Assist with training and mentoring of new staff
✓ Handle and document client concerns and complaints in a timely manner and as appropriate, notify the Supervisors; maintain appropriate documentation

Social Worker & Social Services Worker

Job summary

As part of the Canadian Shield Health Care Services team, the Social Worker is responsible for providing confidential mental health and addiction counselling, education, life-coaching and support to clients and staffs of Canadian Shield Health Care Services. This position operates from a holistic and trauma-informed perspective, is sensitive to the tradition and cultures of many peoples, and provides services required to assist all clients and staffs in attaining and maintaining the highest level of wellness possible.

✓ Provides direct counselling services such as intake, assessment, and holistic treatment planning as appropriate; consultation/liaison with community agencies, schools, hospitals, and other community services as needed or required.
✓ Conducts individual counselling from a trauma-informed perspective.
✓ Maintains basic knowledge of psychotropic medications as prescribed by a physician, maintaining an awareness of the effects of such medications.
✓ Maintains appropriate client documentation.
✓ Promotes and maintains effective working relationships with clients and families, physicians, schools and hospitals to coordinate services, secure input, cooperation, improve client outcomes and provide community support.
✓ Minimum diploma in Social Services Work, or a Bachelor of Social Work Degree (or other Human Services related field) from an accredited college or university and registered with a professional licensing body.
✓ 1 year (recent) direct counselling experience; experience in community development, family support work, group facilitation and/or advocacy is an asset.
✓ Experience in crisis intervention is an asset.
✓ Knowledge of, and demonstrated working experience in, the comprehensive issues relating to substance use and misuse, mental health issues, concurrent disorders, grief/loss, suicide ideation/intent, and family violence.
✓ Demonstrated capacity to develop, implement and evaluate care plans and outcomes of same.

✓ Strong assessment skills relevant to the context of wellness from a holistic approach.
✓ Familiarity with both abstinence and harm reduction model.
✓ Ability to promote healthy lifestyles and healthy choices through personal role-modelling with resources/agencies and other stakeholders.
✓ Familiarity with the concepts of providing services as a life-coach.
✓ Ensure provision of services to clients is completed in a safe environment regardless of location (i.e., home visit, satellite stations or main office).
✓ Strong written and verbal communications skills, which includes correspondence, case notes and report writing.
✓ Ability to work independently and within a multi-disciplinary team.
✓ Demonstrates strong interpersonal, organizational and time management skills.
✓ Functional in Microsoft Office: MSWord, Excel, PowerPoint, Outlook, Publisher.
✓ Ability to work flexible hours that may include some evenings/weekends.
✓ Hold a valid drivers’ license with a clean drivers’ abstract.
✓ This is a designated position of trust working directly with vulnerable persons; this requires a criminal record search – to include vulnerable sector – with clearance is required.
